Live Well – Share Love – Serve others
The App has launched!!!
We offer Health and Wellness solutions for people with the lived experience of HIV virtually in the Healthy on the Move Mobile App (HMMA). The app offers health video content, education, connections to care and promotes social interaction with peers.
HMMA and website is designed to link, educate, and engage those aging with HIV striving for healthier outcomes by focusing on disparities, barriers to care and stigma. The app connects people regardless of gender, age, sexual identity, transmission method or geographic location – PLWH People Living With HIV PERIOD. Our approach is treating each person who is not virally suppressed as part of the most vulnerable population.
Ending the HIV Epidemic
Racial and ethnic minorities have high rates of debilitating disease such as obesity, cancer, diabetes, and AIDS. One of the most glaring health disparities is apparent in the African-American community, where 48% of adults suffer from a chronic disease compared to 39% of the general population. People with HIV live in every community, and they face unique challenges as they age.
Priority Population – funding hasn’t led to increased adherence and regular doctor visits. It is time for a new C.A.S.T. of characters.
The Healthy on the Move mobile app is a resource for people with health conditions providing surveys, assessment forms, linkage to resources, advocate mentorship and a standard of care accountability support networks and HIPPA compliant Tele-mental/Telehealth assessments. The initial health disparities and target communities for the app are PLWH and Substance Use Recovery
